The
Application

The process of becoming a member of the Senior Home
Sharing family may take two to three weeks to complete.
To see if Senior Home Sharing might be a good fit for you
or someone you know, the first step is to call 630/
407-0440, Ext. 251. Our office manager will answer your
questions, provide you with further information and
arrange for you to take a tour.
Once you’ve determined
that a shared senior lifestyle is the right option for
you, the next step is to fill out an application and
financial form. In addition, we request your doctor
complete our physician’s report form. An assessment by a
geriatric social worker and our Director of Resident
Services will be scheduled, with the purpose of
identifying a resident's social and functional needs, in
addition to their cognitive and mental health status. The
social worker identifies whether the resident is
appropriate for group home living, and makes
recommendations for social and health services. After
this interview, the applicant, together with sponsors
(family members, religious advisors, case workers, friends
or others), may decide to move into one of Senior Home
Sharing’s group homes or apartment building.
Upon admission to the
home, each resident is followed regularly by our part-time
Director of Resident Services. The Director of Resident
Services visits with residents on a regular basis to
monitor their needs, and to link them to health and social
services, and activities in the community. She also works
with the families to address any problems that arise and
to assist them when a resident needs to relocate to a more
structured living arrangement.
